To schedule a student's final oral defense, complete this form. When submitted, a copy of the form will be emailed to the Graduate School, the student, and the individual submitting the form. After checking that the student has met all requirements, the Graduate School will email approval or disapproval of the request.

If you encounter any difficulties submitting this form, contact the Graduate School Webmaster to report your problem.

Please submit this completed form no later than two weeks before the oral defense is to be scheduled.
